AI Care at population scale
Notes from our work with national health services — Portugal, Greece, Germany, England
Demand for care has outgrown the hands available to deliver it. The WHO projects Europe will be short of nearly one million health workers by 20301, and there is no workforce plan capable of closing that gap alone. For decades, health systems could only afford to be reactive: wait for a symptom, a referral, an emergency, then respond.
AI changes the math. By introducing AI to complement human care teams, health systems remove the 1:1 structural constraint that has always capped capacity and unlock something bigger than shorter wait times: the ability to make the first move. Screen at-risk populations, prepare them for appointments, follow up automatically, catch signals before they become crises.
Every health system breaks in the same four places — before care (outreach), entering care (the front door), waiting for care (the list), and through care (journey management). This whitepaper gives leaders a scoring framework to diagnose which pressure point to fix first, plus real deployment evidence to build the business case.
Key takeaways inside the guide
- The same four pressure points break every system, and the fixes map directly to them: preventive outreach, the front door, the waiting list, and journey management
- No institution waited for a grand plan — each picked one broken point, ran a first intervention, proved it worked, and scaled from there
- A ready-to-use framework to score your organization's pressure points and decide where to start
- How to build the ROI case by redirecting spend already lost to non-urgent attendances, on-the-day cancellations, and avoidable complications — not by adding new budget
- An inside look at Sword's AI Care model: Phoenix (the AI Care specialist) + human care teams, with clinical oversight at every step
